Guide to obtaining a medical
cannabis card
CARD

Washington STATE

1. To become a medical marijuana patient in the state of Washington, there are several key factors to consider before making an appointment with your chosen medical provider to get your authorization form. You MUST have:

Proof of Residency in Washington State 

(can be any ONE of the following)

  • ONE-year domicile in the state of Washington 

  • Government issued photo ID

  • Residential Lease/Property Deed

  • Utility Bill or Letter from the Government/Court

  • Bank Statement

  • Car Registration

  • Legal Notarized Affidavit of Residence

2. You must also meet certain qualifying health conditions:

  • Cancer, Human Immunodeficiency Virus (HIV), multiple sclerosis, epilepsy or other seizure disorder, or spasticity disorders.

  • Intractable pain limited to: unrelieved by standard medical treatments and medications.

  • Diseases, including anorexia, resulting in nausea, vomiting, wasting, appetite loss, cramping, seizures, muscle spasms and/or spasticity.

  • Chronic renal failure requiring hemodialysis.

  • PTSDo Crohn’s disease with debilitating symptoms unrelieved by standard treatments or medications.

  • Traumatic brain injury

3. If you meet the qualifying health conditions, you must then schedule an appointment with a cannabis-friendly physician. He/she will issue you an Authorization Form

4. Schedule an appointment at a medically endorsed WA marijuana dispensary. Bring your photo ID and the endorsement from your physician along. The dispensary will enter your information into the state database and print your medical card for you (be sure to come to the appointment "camera-ready" as your picture will be taken and printed on your card).

Oregon STATE

1. To become a medical marijuana patient in the state of Oregon, there are several key factors to consider before making an appointment with your chosen medical provider to get your attending physician's statement. You MUST have:

Proof of Residency in Oregon

(can be any ONE of the following):

  • Government issued photo ID

  • Recent utility bill or rental agreement

2. You must also meet certain qualifying health conditions:

  • Cancer

  • Glaucoma

  • A degenerative or pervasive neurological condition

  • HIV/AIDS

  • Post-traumatic stress disorder (PTSD)

  • A medical condition or treatment for a medical condition that produces one or more of the following:

- Cachexia (a weight-loss disease that can be caused by HIV or cancer)
- Severe pain
- Severe nausea
- Seizures, including but not limited to seizures caused by epilepsy
- Persistent muscle spasm, including but not limited to spasms caused by multiple sclerosis

3. If you meet the qualifying health conditions, you must then schedule an appointment with a cannabis-friendly physician. He/she will issue you an "Attending Physician's Statement" or APS.

4. After receiving a completed APS from your physician, you will need to download the OMMP Application Form and Instructions. 

5. After you have completed the form, you will need to mail it along with:

  • Copy of your current proof of Oregon residency

  • Government Issued Photo ID

  • Attending Physician's Statement (APS)

  • Fee payment in the form of Check or Money Order. (Check current rates here.)
